Keraderm MedSpa occasionally offers limited model opportunities for select treatments, educational content, training, portfolio development, and marketing purposes.
If you are interested in being considered as a Keraderm model, please complete the form below. Model appointments are limited and are selected based on treatment needs, provider availability, clinical suitability, and comfort with photo/video consent.
Submitting this form does not guarantee an appointment. All medical aesthetic treatments require proper screening and consultation before proceeding.

Photo & Video Consent for Model Opportunities
As part of a model opportunity at Keraderm MedSpa, photo and/or video documentation may be required before, during, and/or after treatment.
By applying as a model, you understand and agree that Keraderm MedSpa may capture and use your photos, videos, treatment images, and/or testimonials for the following purposes:
- Website content
- Educational material
- Social media posts, reels, stories, and ads
- Marketing and promotional material
- Before-and-after portfolio
- Internal training and treatment documentation
Your images or videos may be used publicly and may appear on Keraderm MedSpa’s website, social media platforms, digital advertising, printed materials, or educational content.
Model opportunities are intended for individuals who are comfortable with photo and/or video use for education, social media, website, and marketing purposes.
A full photo, video, and treatment consent form must be completed and signed prior to treatment.
Required Consent Checkbox
- I understand and agree that, as part of a Keraderm MedSpa model opportunity, photos and/or videos may be taken before, during, and/or after my treatment. I give permission for Keraderm MedSpa to use these photos and/or videos for website content, education, social media, marketing, advertising, portfolio, and training purposes.
- I understand that a full consent form must be completed and signed prior to treatment.
Optional Social Media Tag Consent
- I give Keraderm MedSpa permission to tag my social media account if my content is shared.
Model Deposit Policy
If you are selected for a model appointment, a $75 non-refundable deposit will be required to secure your spot.
This deposit confirms your appointment and reserves the provider’s time. Model appointment availability is limited.
Important Notice
Completing this form does not guarantee that you will be selected as a model.
Model candidates are selected based on the treatment being offered, provider requirements, medical suitability, availability, submitted photos, and willingness to participate in photo and/or video content.
A consultation is required before any injectable, medical aesthetic, or wellness treatment. Keraderm MedSpa reserves the right to decline treatment if it is not clinically appropriate.
If selected, a $75 non-refundable deposit will be requested to secure your model appointment.
